Custom PD request

Complete and submit the online request form below.
An eMINTS staff member will contact you to discuss details for your request within five business days.
eMINTS will send a written summary of your request and how eMINTS proposes to meet your professional development needs for your review within three business days after discussion.
This summary form must be returned to the eMINTS office with an authorized signature no less than three weeks before the scheduled consultation or professional development service date. Note: Any changes to the original summary of services, fees and expectations may incur additional costs.
All requests must be processed and finalized at least three weeks before the requested PD/consultation date(s).
