Step 1 - Secure funding for eMINTS professional development services

Grant applications and information are available for

• Missouri METS School Grants Program
• Title II.D (EETT) in Missouri
• Title I in Missouri
• Title II.D in other states

After grants are awarded or other organization funding is identified, go to step 2.

Step 2 - Register for eMINTS professional development services

Registration for eMINTS professional development services should be submitted not later than June 15, 2009. Requests from schools/districts using grant funds will be accepted after the deadline date if grant awards are not made until after that date. Organizations using grant funds to cover eMINTS participation fees must submit registration within 10 calendar days following notice of grant award.

Participants' contact information

• Project contacts (or their designated proxy) from grant-funded districts must submit participants' contact information to ensure appropriate accounts and access are created for all teachers in PD programs for the 2009-2010 school year.
• All school/district eMINTS instructional specialists who are providing eMINTS comprehensive or eMINTS4All PD training also must submit their participants' information via the online form linked below. This information is required to set up access to eMINTS modules, PD materials (including Tech4Learning resources) and teacher portfolio submission space for comprehensive and eMINTS4All participants.
• Please note that only eMINTS project contacts, PD4ETS participants and certified graduates of the PD4ETS program with approved PD schedules should submit contact information for all teachers they are training -- in one batch.
• Submissions by individual teachers cannot be processed.

Replacement teachers

Professional development registration for replacement teachers in existing eMINTS and eMINTS4All classrooms should be made as soon as the replacement teacher is assigned. If replacement(s) have not been identified when the online registration form is submitted, please submit an additional form and flag it as the addition of more teachers in the Notes field at the end of the form.

Each organization/district is required to provide the following information:

1. Organization/district contact information.
2. Contact information for the organization/district's official eMINTS project contact person.
3. Contact information for district/organization instructional specialist(s) (if applicable).
4. Contact information for each person who is registering for eMINTS programs and services in FY10:
1. Legal first name
2. Nickname or other name used, if other than legal first name (optional)
3. Last name
4. E-mail address
5. School building name

Submit information online

Please use the electronic form linked below to submit all information for FY10 eMINTS professional development services and participants registering for eMINTS PD programs, both year 1 and year 2. The eMINTS National Center will prepare a contract for professional development services or access fees with your organization/district and create accounts for participants' access to eMINTS online learning environments based on the information submitted.

3. To ensure accurate submission, do NOT use the "Back" button in your browser to navigate through the form; use the Next link at the bottom of each screen.
4. If you have more participants than form fields available for any program, please submit additional forms and note the additional form in the Notes field at the end of the submission form.
